Billing Services

Your Memorial Medical Group bill includes charges for goods and services rendered by your physician. Depending on your care, you may receive separate bills for services such as x-ray or lab work.

You may contact our financial services department at (337) 494-3265 with any questions or concerns about your bill or to pay your bill by phone. You may also visit the billing and financial services office located at 3050 Aster Street in Lake Charles. It is open Monday – Friday from 8:00 am to 4:30 pm.

When corresponding with the Business Office, please provide your full name, daytime and evening telephone number, account number, and a brief summary regarding any information you wish to provide to us or the nature of your question. A financial service representative will follow-up with you as appropriate within two business days.

Memorial Medical Group will bill your insurance(s) prior to billing you for any balances left on your account. However, if your insurance fails to pay the claims, you will be responsible for any and all costs incurred with the provision of your care. All balances are expected to be paid in full within 30 days of being billed, or you must make appropriate payment arrangements if that is not possible.

Health care services may be provided to you at a network healthcare facility by facility-based physicians who are not in your health plan. You may be responsible for payment of all or part of the fees for those out-of-network services, in addition to applicable amounts due for co-payments, coinsurance, deductibles, and non-covered services. Specific information about in-network and out-of-network facility based physicians can be found at the website address of your health plan or by calling the customer service telephone number of your health plan.

Special Circumstances

Please let us know of any special circumstance that may impact the timely, accurate and complete payment for your services. Our financial service representatives are able and willing to accommodate you, but we need to know of special circumstances in advance of your service.

Payment Options

Memorial’s basic payment plan can be arranged after an interview with one of our patient account representatives. Once a payment plan has been authorized, failure to make regular and routine payments may result in the debt being transferred to a collection agency.

Medicaid Application Center

Memorial does have a Medicaid Application Center in the Patient Access Center, located in the Atrium of our main campus. The office hours are 8:00 am to 4:30 pm. Application requests are by appointment only. Please call (337) 494-4637 to set up, cancel or reschedule an appointment.

Financial Assistance

Memorial is committed to extending financial assistance, and free or reduced fee care, to qualifying individuals. If payment on your health care expenses could create a financial hardship for you, please contact a financial service representative, who will help you apply for financial assistance. Applications will be processed and approval will be determined based on specific criteria. Information you provide is confidential and is only reviewed by the staff processing your application. If approved, your financial obligation will be reduced. We will not discriminate in the determination of financial eligibility on the basis of race, color, creed, sex, age or handicap.
